An NHS watchdog has placed a Leicestershire hospital in special measures after staff failed to admit their mistakes and apologise over the death of a patient.
The Burton Park hospital, in Melton, was found to have recorded 24 serious incidents in the last six months when it was visited by the Care Quality Commission.
Inspectors also found managers failed to act on allegations of abuse on patients, staff claims they suffered from bullying, harassment and discrimination. As we see in our cases, a culture of openness and willingness to learn from mistakes is so important, so that errors and problems are identified and action taken, so that they are not repeated.
